By signing this document, I, the undersigned participant or the parent/guardian of a minor participant (the "Participant"), acknowledge and agree to the following terms regarding participation in programs, camps, clinics, or any related activities (the "Activities") provided by Spotlight Athletic Development Inc. (“Spotlight”):

1. Acknowledgment of Risk

I understand that participating in Spotlight’s Activities involves inherent risks, including but not limited to the risk of injury, illness, permanent disability, or death.
I voluntarily assume all risks associated with participation, whether arising from physical activity, the use of equipment, or other participants.

2. Release of Liability

I agree to release, indemnify, and hold harmless Spotlight, its owners, directors, employees, agents, coaches, assistant coaches, and volunteers from any and all claims, liabilities, damages, or demands arising from or related to participation in the Activities.
This includes claims resulting from the ordinary negligence of Spotlight or its representatives but excludes gross negligence or intentional misconduct.

3. Agreement Not to Sue

I agree not to pursue legal action against Spotlight or its representatives for any injury, illness, or damages sustained during participation in the Activities, except in cases of gross negligence or intentional misconduct.

4. Medical Authorization

In the event of a medical emergency, I authorize Spotlight staff to seek appropriate medical care for myself or my child.
I understand and accept responsibility for all medical costs incurred as a result of such care.

5. Photo and Media Consent

I grant permission to Spotlight to capture and use photographs, videos, or other media of myself or my child taken during participation in the Activities.
I understand these materials may be used for promotional purposes, including but not limited to Spotlight’s website, social media, and advertising.
If I wish to withhold consent for media use, I will notify Spotlight in writing prior to participation.

6. Compliance with Rules

I agree that the Participant must adhere to all Spotlight rules, policies, and instructions from staff during participation.
Spotlight reserves the right to remove participants for inappropriate behavior or rule violations without refund.

7. Cancellation and Refund Policy

I understand that cancellations must be made at least 7 days in advance of the scheduled session or program start date to qualify for a refund.
Cancellations made with less than 7 days’ notice are not eligible for refunds.

8. Online Waiver Requirement

I acknowledge that this waiver must be completed and submitted prior to participating in any Spotlight Activities. Failure to complete this waiver may result in forfeiture of the session or program without refund.

9. Governing Law and Severability

This agreement is governed by the laws of the Province of Alberta. Any disputes will be resolved exclusively in Alberta courts.
If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in effect.
